HIV disease

Human immunodeficiency virus (HIV) is a blood-borne, sexually transmissible virus (see the image below.) The virus is typically transmitted via sexual intercourse, shared intravenous drug paraphernalia, and mother-to-child transmission (MTCT), which can occur during the birth process or during breastfeeding.

Ewing, Jr. The most common route of infection varies from country to country and even among cities, reflecting the population in which HIV was introduced initially and local practices. Co-infection with other viruses that share similar routes of transmission, such as hepatitis B, hepatitis C, and human herpes virus 8 (HHV8; also known as Kaposi sarcoma herpes virus [KSHV]), is common.
Two distinct species of HIV (HIV-1 and HIV-2) have been identified, and each is composed of multiple subtypes, or clades. All clades of HIV-1 tend to cause similar disease, but the global distribution of the clades differs. This may have implications on any future vaccine, as the B clade, which is predominant in the developed world (where the large pharmaceutical companies are located), is rarely found in the developing countries that are more severely affected by the disease.
HIV-1 probably originated from one or more cross-species transfers from chimpanzees in central Africa.[1] HIV-2 is closely related to viruses that infect sooty mangabeys in western Africa.[2] Genetically, HIV-1 and HIV-2 are superficially similar, but each contains unique genes and its own distinct replication process.
HIV-2 carries a slightly lower risk of transmission, and HIV-2 infection tends to progress more slowly to acquired immune deficiency syndrome (AIDS). This may be due to a less-aggressive infection rather than a specific property of the virus itself. Persons infected with HIV-2 tend to have a lower viral load than people with HIV-1[3, 4] , and a greater viral load is associated with more rapid progression to AIDS in HIV-1 infections.[5, 6]
HIV-2 is rare in the developed world. Consequently, most of the research and vaccine and drug development has been (perhaps unfairly) focused on HIV-1.

Initial description and early spread

In the United States, HIV disease was first described in 1981 among 2 groups, one in San Francisco and the other in New York City. Numerous young homosexual men presented with opportunistic infections that, at the time, were typically associated with severe immune deficiency: Pneumocystis pneumonia (PCP) and aggressive Kaposi sarcoma.[7]
HIV itself was not identified for another 2 years.[8] During that time, various other causes were considered, including lifestyle factors, chronic drug abuse, and other infectious agents.[9] The HIV epidemic spread rapidly and silently in the absence of testing.
However, clear clinical implications arose before society became aware of the disease; for example, prior to the recognition of HIV, only one case of Pneumocystis pneumonia not clearly associated with immune suppression was diagnosed in the United States between January 1976 and June 1980. In 1981 alone, 42 similar diagnoses were made, and by December 1994, 127,626 cases of Pneumocystis pneumonia with HIV infection as the only identified cause of immune suppression had been reported to the Centers for Disease Control and Prevention (CDC). Also, Kaposi sarcoma is up to 30,000 times more likely to develop in persons with HIV infection than in immunocompetent persons.
The spread of HIV was retrospectively shown to follow the trucking routes across Africa from logging camps, and the bush-meat trade combined with aggressive logging and improved transportation in the mid-20th century may have allowed what was likely occasional cross-species transmission events to propagate across the country and, eventually, the globe.[10]

Stigma of HIV infection

A considerable amount of stigma has been attached to HIV infection, mostly because of the virus's association with sexual acquisition and the inference of sexual promiscuity. Consequences of this stigma have included discrimination and reluctance to be tested for HIV infection. The stigma of HIV infection is also associated with a fear of acquiring a rapidly fatal infection from relatively casual contact.
Such attitudes are inappropriate because HIV is poorly transmissible without sexual contact or blood contact. In addition, the expected survival is long in patients with HIV infection who are receiving treatment. HIV is not transmitted during casual contact and is readily inactivated by simple detergents. Much of the concern regarding HIV infection is due to the incurability of the infection and the relentless immune decline and eventual premature death in the vast majority of infected people.

AIDS denialism

A small but vocal minority of people, including some scientists, continue to argue that HIV does not exist, or does not cause AIDS, and that the HIV tests are unreliable or that the therapies are toxic. Such misinformation is usually based on a lack of understanding of the scientific literature, deliberate misrepresentation, or logical fallacies based on pseudoscientific arguments.
All of the arguments proposed by these dissenters have been addressed and rebutted in the scientific literature and public discussion and even tested and rejected in the legal system. Nevertheless, they persist, and such views can have extremely harmful effects on people who are exposed to HIV infection unnecessarily or who refuse treatment for their progressing infection.
Clinicians should be aware of these issues, should be able and willing to address misinformation, and should direct their patients to reliable sources of information.
Political denial and inaction have also likely caused considerable damage. Several governments in countries with high HIV infection rates were slow to admit that they had an HIV epidemic, and at least one (South Africa) initially rejected that AIDS was even a problem, then that the disease was caused by HIV infection, and, most recently, that antiretroviral therapy was effective in treating HIV infection and preventing MTCT. Changes have now occurred but have been slow and have cost hundreds of thousands of lives.

The quest for understanding of HIV

Since the discovery of HIV and its link to AIDS, great strides have been made in understanding its biology and in developing effective treatments. The difficulty in dealing with HIV on a global scale is largely due to the fact that HIV infection is far more common in resource-poor countries.
In the developed world, antiretroviral therapy has greatly improved prognosis and increased survival rates. Public education programs have raised awareness such that testing and prevention of infection are more common. Both of these approaches are difficult in countries with undereducated or underfunded populations.

The document was originally written in September 1995, prior to the advent of highly active antiretroviral therapy (HAART), which has significantly improved AIDS-free survival in persons infected with HIV. This version was updated March 2010.

Patient confidentiality

HIV-related health information is typically considered separate from other health information and may require separate consent to share or divulge.
Health care workers who are infected with HIV may be required to divulge their status to their employer or patients and may be restricted in the types of procedures they can perform.

Fobia adalah rasa ketakutan yang berlebihan pada sesuatu hal atau fenomena.
Fobia bisa dikatakan dapat menghambat kehidupan orang yang mengidapnya. Bagi sebagian orang, perasaan takut seorang pengidap Fobia sulit dimengerti. Itu sebabnya, pengidap tersebut sering dijadikan bulan bulanan oleh teman sekitarnya.
Ada perbedaan "bahasa" antara pengamat fobia dengan seorang pengidap fobia. Pengamat fobia menggunakan bahasa logika sementara seorang pengidap fobia biasanya menggunakan bahasa rasa. Bagi pengamat dirasa lucu jika seseorang berbadan besar, takut dengan hewan kecil seperti kecoak atau tikus. Sementara di bayangan mental seorang pengidap fobia subjek tersebut menjadi benda yang sangat besar, berwarna, sangat menjijikkan ataupun menakutkan.
Dalam keadaan normal setiap orang memiliki kemampuan mengendalikan rasa takut. Akan tetapi bila seseorang terpapar terus menerus dengan subjek Fobia, hal tersebut berpotensi menyebabkan terjadinya fiksasi. Fiksasi adalah suatu keadaan dimana mental seseorang menjadi terkunci, yang disebabkan oleh ketidak-mampuan orang yang bersangkutan dalam mengendalikan perasaan takutnya. Penyebab lain terjadinya fiksasi dapat pula disebabkan oleh suatu keadaan yang sangat ekstrem seperti trauma bom, terjebak lift dan sebagainya.
Seseorang yang pertumbuhan mentalnya mengalami fiksasi akan memiliki kesulitan emosi (mental blocks) dikemudian harinya. Hal tersebut dikarenakan orang tersebut tidak memiliki saluran pelepasan emosi (katarsis) yang tepat. Setiap kali orang tersebut berinteraksi dengan sumber Fobia secara otomatis akan merasa cemas dan agar "nyaman" maka cara yang paling mudah dan cepat adalah dengan cara "mundur kembali"/regresi kepada keadaan fiksasi. Kecemasan yang tidak diatasi seawal mungkin berpotensi menimbulkan akumulasi emosi negatif yang secara terus menerus ditekan kembali ke bawah sadar (represi). Pola respon negatif tersebut dapat berkembang terhadap subjek subjek fobia lainnya dan intensitasnya semakin meningkat. Walaupun terlihat sepele, “pola” respon tersebut akan dipakai terus menerus untuk merespon masalah lainnya. Itu sebabnya seseorang penderita fobia menjadi semakin rentan dan semakin tidak produktif. Fobia merupakan salah satu dari jenis jenis hambatan sukses lainnya.

Morfologi :
-cacing betina berukuran 8-13 mm x 0,4mm, pada ujung anterior ada penebalan kutikulum seperti sayap (alae)
- cacing jantan 2-5 mm mempunyai sayap dekornya melingkar, bentuknya seperti tanda tanya (?)

SIKLUS HIDUP
Cacing dewasa hidup di sekum, usus besar dan di usus halus yang berdekatan dengan sekum. Mereka memakan isi usus penderitanya.
Perkawinan (atau persetubuhan) cacing jantan dan betina kemungkinan terjadi di sekum. Cacing jantan mati setelah kawin dan cacing betina mati setelah bertelur. Cacing betina yang mengandung 11.000-15.000 butir telur akan bermigrasi ke daerah sekitar anal (perianal) untuk bertelur. Migrasi ini berlangsung 15 – 40 hari setelah infeksi. Telur akan matang dalam waktu sekitar 6 jam setelah dikeluarkan, pada suhu tubuh. Dalam keadaan lembab telur dapat hidup sampai 13 hari.

Patologi dan gejala klinis
-
Iritasi di sekitar anus, perienum dan vagina oleh cacing betina gravid yang bermigrasi ke daerah anus dan vagina sehingga menyebabkan pruritus lokal
- kurang nafsu makan, berat  badan turun, aktivitas meninggi, enuresis, cepat marah, gigi menggeretak, insomnia, dan mastrubasi,tetapi kadang sulit untuk membuktikan sebab cacing kremi
Pengobatan
-
obat piperazin dosis tunggal 3-4 gram(dewasa) atau 25mg/kg berat  badan (anak-anak) diminum dengan segelas air pada pagi hari sehingga obat sampai di sekum dan kolon.
- pirantel pamoat dosis 10 mg/ kg berat badan
- mebendazol dosis tunggal 100 mg
- albendazol dosis tunggal 400 mg
pencegahan dengan kebersihan lingkungan dan selalu cuci tangan sebelum makan
Pencegahan
1. Mencuci tangan sebelum makan dan setelah buang air besar
2.Memotong kuku dan menjaga kebersihan kuku
3.Mencuci seprei minimal 2 kali/minggu
4.Mencuci jamban setiap hari
5.Menghindari penggarukan daerah anus karena bisa mencemari jari-jari tangan dan setiap benda yang dipegang/disentuhnya
6.Menjauhkan tangan dan jari tangan dari hidung dan mulut
Infeksi dan penularan
  1. Penularan dari tangan ke mulut (hand to mouth), setelah anak – anak menggaruk daerah sekitar anus oleh karena rasa gatal, kemudian mereka memasukkan tangan atau jari – jarinya ke dalam mulut. Kerap juga terjadi, sesudah menggaruk daerah perianal mereka menyebarkan telur kepada orang lain maupun kepada diri sendiri karena memegang benda-benda maupun pakaian yang terkontaminasi.
  2. Debu merupakan sumber infeksi oleh karena mudah diterbangkan oleh angin sehingga telur yang ada di debu dapat tertelan.
  3. Anjing dan kucing bukan mengandung cacing kremi tetapi
    dapat menjadi sumber infeksi oleh karena telur dapat menempel pada bulunya.

What is diabetes? What causes diabetes?

Diabetes (diabetes mellitus) is classed as a metabolism disorder. Metabolism refers to the way our bodies use digested food for energy and growth. Most of what we eat is broken down into glucose. Glucose is a form of sugar in the blood - it is the principal source of fuel for our bodies.

When our food is digested the glucose makes its way into our bloodstream. Our cells use the glucose for energy and growth. However, glucose cannot enter our cells without insulin being present - insulin makes it possible for our cells to take in the glucose.

Insulin is a hormone that is produced by the pancreas. After eating, the pancreas automatically releases an adequate quantity of insulin to move the glucose present in our blood into the cells, and lowers the blood sugar level.

A person with diabetes has a condition in which the quantity of glucose in the blood is too elevated (hyperglycemia). This is because the body either does not produce enough insulin, produces no insulin, or has cells that do not respond properly to the insulin the pancreas produces. This results in too much glucose building up in the blood. This excess blood glucose eventually passes out of the body in urine. So, even though the blood has plenty of glucose, the cells are not getting it for their essential energy and growth requirements.

Why is it called Diabetes Mellitus?

Diabetes comes from Greek, and it means a siphon. Aretus the Cappadocian, a Greek physician during the second century A.D., named the condition diabainein. He described patients who were passing too much water (polyuria) - like a siphon. The word became "diabetes" from the English adoption of the Medieval Latin diabetes.
In 1675 Thomas Willis added mellitus to the term, although it is commonly referred to simply as diabetes. Mel in Latin means honey; the urine and blood of people with diabetes has excess glucose, and glucose is sweet like honey. Diabetes mellitus could literally mean "siphoning off sweet water".
In ancient China people observed that ants would be attracted to some people's urine, because it was sweet. The term "Sweet Urine Disease" was coined.

There are three main types of diabetes:

  • Diabetes Type 1 - You produce no insulin at all.
  • Diabetes Type 2 - You don't produce enough insulin, or your insulin is not working properly.
  • Gestational Diabetes - You develop diabetes just during your pregnancy.

Diabetes Types 1 and 2 are chronic medical conditions - this means that they are persistent and perpetual. Gestational Diabetes usually resolves itself after the birth of the child.

Treatment is effective and important

All types of diabetes are treatable, however Type 1 and Type 2 diabetes last a lifetime; there is no known cure. The patient receives regular insulin, which became medically available in 1921. The treatment for a patient with Type 1 is mainly injected insulin, plus some dietary and exercise adherence.

Patients with Type 2 diabetes are usually treated with tablets, exercise and a special diet, but sometimes insulin injections are also required.

If diabetes is not adequately controlled the patient has a significantly higher risk of developing complications, such as hypoglycemia, ketoacidosis, and nonketotic hypersosmolar coma. Longer term complications could be cardiovascular disease, retinal damage, chronic kidney failure, nerve damage, poor healing of wounds, gangrene on the feet which may lead to amputation, and erectile dysfunction.

DEFINISI
Dismenore adalah nyeri perut yang berasal dari kram rahim dan terjadi selama menstruasi.

PENYEBAB
Disebut dismenore primer jika tidak ditemukan penyebab yang mendasarinya dan dismenore sekunder jika penyebabnya adalah kelainan kandungan.

Dismenore primer sering terjadi, kemungkinan lebih dari 50% wanita mengalaminya dan 15% diantaranya mengalami nyeri yang hebat.
Biasanya dismenore primer timbul pada masa remaja, yaitu sekitar 2-3 tahun setelah menstruasi pertama.

Nyeri pada dismenore primer diduga berasal dari kontraksi rahim yang dirangsang oleh prostaglandin.
Nyeri dirasakan semakin hebat ketika bekuan atau potongan jaringan dari lapisan rahim melewati serviks (leher rahim), terutama jika saluran serviksnya sempit. Faktor lainnya yang bisa memperburuk dismenore adalah:
- rahim yang menghadap ke belakang (retroversi)
- kurang berolah raga
- stres psikis atau stres sosial.

Pertambahan umur dan kehamilan akan menyebabkan menghilangnya dismenore primer.
Hal ini diduga terjadi karena adanya kemunduran saraf rahim akibat penuaan dan hilangnya sebagian saraf pada akhir kehamilan.

Perbedaan beratnya nyeri tergantung kepada kadar prostaglandin. Wanita yang mengalami dismenore memiliki kadar prostaglandin yang 5-13 kali lebih tinggi dibandingkan dengan wanita yang tidak mengalami dismenore.
Dismenore sangat mirip dengan nyeri yang dirasakan oleh wanita hamil yang mendapatkan suntikan prostaglandin untuk merangsang persalinan.

Dismenore sekunder lebih jarang ditemukan dan terjadi pada 25% wanita yang mengalami dismenore.
Penyebab dari dismenore sekunder adalah:
  • Endometriosis
  • Fibroid
  • Adenomiosis
  • Peradangan tuba falopii
  • Perlengketan abnormal antara organ di dalam perut.
  • Pemakaian IUD.
    Dismenore sekunder seringkali mulai timbul pada usia 20 tahun.

  • GEJALA
    Dismenore menyebabkan nyeri pada perut bagian bawah, yang bisa menjalar ke punggung bagian bawah dan tungkai.
    Nyeri dirasakan sebagai kram yang hilang-timbul atau sebagai nyeri tumpul yang terus menerus ada.

    Biasanya nyeri mulai timbul sesaat sebelum atau selama menstruasi, mencapai puncaknya dalam waktu 24 jam dan setelah 2 hari akan menghilang.
    Dismenore juga sering disertai oleh sakit kepala, mual, sembelit atau diare dan sering berkemih. Kadang sampai terjadi muntah.

    DIAGNOSA
    Diagnosis ditegakkan berdasarkan gejala dan hasil pemeriksaan fisik.

    PENGOBATAN
    Untuk mengurangi rasa nyeri bisa diberikan obat anti peradangan non-steroid (misalnya ibuprofen, naproxen dan asam mefenamat). Obat ini akan sangat efektif jika mulai diminum 2 hari sebelum menstruasi dan dilanjutkan sampai hari 1-2 menstruasi.
    Selain dengan obat-obatan, rasa nyeri juga bisa dikurangi dengan:
    - istirahat yang cukup
    - olah raga yang teratur (terutama berjalan)
    - pemijatan
    - yoga
    - orgasme pada aktivitas seksual
    - kompres hangat di daerah perut.

    Untuk mengatasi mual dan muntah bisa diberikan obat anti mual, tetapi mual dan muntah biasanya menghilang jika kramnya telah teratasi.
    Gejala juga bisa dikurangi dengan istirahat yang cukup serta olah raga secara teratur.

    Jika nyeri terus dirasakan dan mengganggu kegiatan sehari-hari, maka diberikan pil KB dosis rendah yang mengandung estrogen dan progesteron atau diberikan medroxiprogesteron.
    Pemberian kedua obat tersebut dimaksudkan untuk mencegah ovulasi (pelepasan sel telur) dan mengurangi pembentukan prostaglandin, yang selanjutnya akan mengurangi beratnya dismenore.
    Jika obat ini juga tidak efektif, maka dilakukan pemeriksaan tambahan (misalnya laparoskopi).

    Jika dismenore sangat berat bisa dilakukan ablasio endometrium, yaitu suatu prosedur dimana lapisan rahim dibakar atau diuapkan dengan alat pemanas.

    “The Domino Tattoo” by Cyrian Amberlake (ebook)
    I read this book based on a recommendation from author Portia Da Costa. It is erotica rather than erotic romance and follows one woman’s strange sexual journey. What I liked best about this story is the psychological aspects. Josephine is having some difficulties after her divorce and goes to a family doctor for help. She is sent on a journey without ever really knowing what exactly is happening. This book is a great read, and I will be reading the next two books in the series. Big thanks to Portia Da Costa for the recommendation!

    This book releases on July 3rd, 2012, review to come. But as a hint, this is a great read! My first Dee Tenorio, but not my last.

    In this erotic romance, a couple engages in some steamy role playing. The heroine plays wayward student to the hero’s stern teacher. Some readers may want to be aware of the role playing, as the student/teacher dynamic does flirt with non-consensuality. I enjoyed this story and think that it was quite well done. Lapthorne sets the scene for the role playing masterfully. This is a very hot read.
